Our commitment
VYTA Healthcare and its participating providers are required by law to maintain the privacy of protected health information (PHI) and to provide you with this notice of our legal duties and privacy practices.
How we may use PHI
For treatment (coordinating your care across providers), payment (billing and eligibility), and healthcare operations (quality review, provider training, platform safety).
Disclosures requiring authorization
Marketing, the sale of PHI, and most uses of psychotherapy notes require your written authorization, which you may revoke at any time.
Your rights
You have the right to inspect and copy your records, request corrections, request restrictions on certain uses, request confidential communications, and receive an accounting of disclosures.
Breach notification
You will be notified promptly if a breach compromises the privacy or security of your PHI.
Complaints
You may file a complaint with us at info@vytahealthcare.com or with the U.S. Department of Health and Human Services. We will never retaliate for filing a complaint.
